We invite you to print and complete your patient forms from the comfort of your own home or arrive 15 minutes early to your scheduled appointment to complete needed forms. All new patients will need a completed health history, HIPAA acknowledgment, and a signed credit policy. If you have current radiographs (x-rays) from another dental office, we would love to request those for you to save you time and money. Please print and complete the Radiograph Request Form below and drop it by our office or email it to us two business days prior to your dental appointment. Thank you!
